I am 73 right now and received my AMS 700 CX in 2018. I have been an AMS champion for the last 5 years. Helping men decide on the IPP. ED, Shockwave, Stem cell Therapy - can I safely transition from Radial to Focused Treatment?

Hi Dr. Malik, I'm Bob from Canada. Firstly, I love your videos and podcasts, keep up the awesome work and thank you for giving so many Men (and subsequently women) a platform to discuss their Sexual health, Intimacy is such an important part of a successfully relationship, this should be a top agenda item for couples to discuss, but unfortunately its not. I’m a 52 yr old Male. I discovered in January 2022 that I had a high blood sugar reading A1C of 9.4. At the time I was very heavy (290lbs), and I’m only 5 ft 11. My doctor at the time felt that all other lab results seemed absolutely perfect, and so he concluded I’d only been having high blood sugars very briefly (perhaps 6 months at most). He gave me specific instructions on what to do to mitigate any complications. This included: 1. Lose 100 lbs and 2. Start taking metformin until My weight was in a manageable range and hopefully then I could consider coming off of the medication. Within 6 months (End of July 2022) I’d lost slightly more than 60 lbs (I weighed then 226 lbs), so I asked to be taken off of metoformin. I felt that I would do my best to manage the higher than normal blood sugars with diet and exercise along. The Doctor (a different Doctor), agreed to put me on a ‘medication holiday’ and the result was average A1C’s between 5.7 and 5.9 from about July 2022 to August 2024 (while not on Metformin). Not great, but diet and exercise were still keeping me inside of what seemed a reasonable range. I now weigh 192 lbs. In Jan 2024, feeling my better about my health, became romantically involved with a new girlfriend. Unfortunately I didn’t rise to the occasion when we started getting intimate, which seemed very unusual to me. My GF (now ex GF, yes she broke up with me over this) insisted I had ED. Most importantly, I started to investigate further, talked with my Doctor, and began to notice some discomfort around the perineum nerve. I also sustained an Groin injury summer 2018. While playing with my 10 year old Nephew, I attempted to slide down a Kiddle slide at a playground and landed very Violently in a seated position against a sandbar located at the bottom of the Slide. The impact almost completely knocked me out. So there are two(2) things going on here. 1. A brief history of higher than average Blood sugars (I’m not sure for how long, per my Doctor he thinks it was only 6 months), and 2. An Injury from 2018. The good news being that with regular exercise, significant weight loss, I have turned it around rather quickly (Blood sugars back in a reasonable range and life seems to be getting back to normal – except for the possibility of what seems to be mild ED)! My Question(s): 1. Do I have ED? I discussed these matters with my Dr. who requested an ultrasound. It revealed what was described as “Punctate calcification within penile bulb, a nonspecific finding. No focal tenderness.” The Impression revealed “Unremarkable sonographic assessment of the scrotum and perineal region. No specific finding to account for patient's symptoms. Consider pudendal neuropathy as a potential cause of patient's symptoms, given distribution of symptoms and history of trauma.”. Could you possibly explain what this might mean in terms of the Penile Bulb? I’m not sure that I completely understand? Does this indicate that I have ED (I’m not sure, nobody is explaining this to me)? I can still get an erection, its just nothing like it was when I was in my 20’s, and I don’t know if it is enough for penetrative intercourse. How can a guy determine this w/o getting himself back into another embarrassing situation with a woman, like what happened in March 2024 between me and my now Ex GF? Is there some sort of test for this (erection hardness, bloodflow, vascularization, etc etc)? If there are some small markers of “Calcification” does this mean that it continues to get worse throughout the tissue over time? Is Stem Cell therapy an option? 2. Shockwave Therapy: I watched your recent Podcast about Shockwave therapy for ED. You described the difference between Radial Shockwaves and Low Intensity Shock Wave Therapy (LiSWT). I recently did a ‘Gainswave session’ at a nearby Men’s Sexual health clinic – there were 7 treatments involved at $500.00/Session. However, I noticed once I completed the sessions (by Oct 10th 2024) that the Doctor was using what is called a “Medical Stores D-Actor” Medical pulse generator, to generate the waves. This is a ‘Radial Wave’ therapy device, NOT a focused low intensity device. I paid $3000.00 out of my own pocket for these treatments. It was expensive, but I felt my health was worth it, and I didn’t know the difference until I was finally done my treatments. I didn’t see your video until just this past week. Except now, after watching your video, I see that the ‘Medical Stores SD-1’ is the proper Focused Shock Wave device to be using, as it carries the proper waveform and amplitude that most promises ED recovery. The Question I have is, I just recently completed the Radial Shockwave therapy Oct 10th, 2024. I’m wondering, if I should instead, consider doing another set of treatments (at a whopping additional $4000.00) with the proper Clinic that works with the Medical Stores SD-1 Device that you mention in your video. Is there any risk of having these treatments so close together like this? Could it possibly damage any Penile tissue by doing so? 3. After hearing these Questions, What are my odds of a full recovery. I’ve been healthy all my life up to about age 46 or 47….when I started gaining weight, and didn’t notice issues with blood sugars until about age 50 (when I quickly intervened). Of course COVID did not help either, you couldn’t pay a Million dollars to get into a public Gym at he time, b/c everyone was in Lockdown. The Gym used to be a regular thing for me, and this precisely coincided with my Rapid weight Gain and higher than average blood sugars, and subsequent onset of what may possibly be mild ED. I now Run every morning at 5:30am for 30 mins and combine it with resistance exercises, I look and feel pretty good for a 52 year old guy, and still turn the ladies heads, that’s a nice thing to still have even at my age! I was just talking with a 35 year old lady today. While there’s an age gap, she’s still interested, I must be doing something right. However, I still struggle with these recent complications. If I have mild ED, is my dating life now over? Never married, no kids! 4. I suppose if I have ED, then is Stem Cell therapy replacement an option for me? Have you heard of this being done and where can I find out more information about it? Are there any clinical trials that are presently underway for it? 5. On top of all of this, my Doctor has now indicated to me that I have what is called Fatty Liver. He’s told me specifically, you need exercise! A lot of it!
